    That blade looks kinda rusty. Not a very sanitary tool to use. The guy on the ground could end up with a nasty infection.
    I don't think it's for surgical purposes. I think he's just getting a haircut.
    In that case, he should look for a new stylist. Hygiene and sanitation are more important than anything else when it comes to getting your hair done.
