TBF, they need to do a whole comprehensive review of every element that could have contributed to the crash and assess whether any changes are needed to prevent similar accidents in the future. Were there any mechanical issues? Medical issues? Drugs/alcohol? Were they properly trained and certified? It's just in this case it was entirely human error. The Cessna was at a lower altitude and began climbing. The De Havilland probably thought the Cessna would fly under him and didn't think to maneuver. -
